Briefing before pilotage, restricted visibility or a busy traffic scheme means the whole team knows the plan, the expected hazards, and what should trigger a call to the master. Workload is actively managed, splitting navigation, lookout, communications and paperwork between people so no one is simultaneously plotting a fix, answering the radio and keeping lookout in heavy traffic, a combination that reliably produces missed information.
A defining principle is that rank does not override observation: a junior officer, cadet or helmsman who notices something wrong, a target closing faster than expected, a depth that does not match the chart, is trained to speak up immediately, and the officer in charge is trained to listen and check rather than dismiss it, because many serious accidents trace back to someone noticing a problem and staying quiet out of deference to rank. Cross-checking is built into routine rather than left to chance: a GPS position is checked against radar ranges or a visual fix where possible, an ARPA target's CPA is sanity-checked against what the eye reports, and no single source, however normally reliable, is trusted blindly. Since BRM became embedded in STCW training, serious incident investigations now routinely examine bridge team performance and communication alongside the technical cause, because in most collisions and groundings the equipment worked as designed and the failure was in how the team used it.
